Discussion: Presidential Agendas

Regardless of political affiliation, every citizen has a stake in healthcare policy decisions. Hence, it is little wonder why healthcare items become such high-profile components of presidential agendas. It is also little wonder why they become such hotly debated agenda items.

Consider a topic that rises to the presidential level. How did each of the presidents (Trump, Obama, and Bush) handle the problem? What would you do differently?

To Prepare:

  • Review the Resources and reflect on the importance of agenda setting.
  • Consider how federal agendas promote healthcare issues and how these healthcare issues become agenda priorities.

 It may seem to you that healthcare has been a national topic of debate among political leaders for as long as you can remember.

Healthcare has been a policy item and a topic of debate not only in recent times but as far back as the administration of the second U.S. president, John Adams. In 1798, Adams signed legislation requiring that 20 cents per month of a sailor’s paycheck be set aside for covering their medical bills. This represented the first major piece of U.S. healthcare legislation, and the topic of healthcare has been woven into presidential agendas and political debate ever since.

As a healthcare professional, you may be called upon to provide expertise, guidance and/or opinions on healthcare matters as they are debated for inclusion into new policy. You may also be involved in planning new organizational policy and responses to changes in legislation. For all of these reasons you should be prepared to speak to national healthcare issues making the news.

In this Assignment, you will analyze recent presidential healthcare agendas. You also will prepare a fact sheet to communicate the importance of a healthcare issue and the impact on this issue of recent or proposed policy.

To Prepare:

  • Review the agenda priorities of the current/sitting U.S. president and the two previous presidential administrations.
  • Select an issue related to healthcare that was addressed by each of the last three U.S. presidential administrations.
  • Reflect on the focus of their respective agendas, including the allocation of financial resources for addressing the healthcare issue you selected.
  • Consider how you would communicate the importance of a healthcare issue to a legislator/policymaker or a member of their staff for inclusion on an agenda.

Be sure to address the following:

  • Identify and provide a brief description of the population health concern you selected and the factors that contribute to it.
  • Describe the administrative agenda focus related to the issue you selected.
  • Identify the allocations of financial and other resources that the current and two previous presidents dedicated to this issue.
  • Explain how each of the presidential administrations approached the issue.

Along with physiological health, mental health should receive crucial attention from healthcare providers. People go through various stressful situations which has a direct impact on their health. There can be both personal and global factors contributing to mental health issues.

 

Considering the interdependence between physiological and mental health, the topic should receive presidential attention. If the majority of population face mental health issues then the country’s economy and people’s social life will be highly impacted. Consequently, the government has two responsibilities – enact policies that will help people facing mental health issues and develop mechanisms to minimize factors that have direct negative impact on population mental health.

Up till 2019, President Trump was decreasing funding for Substance Abuse and Mental Health Services Administration’s program directed to Mental health, directed focus to other healthcare areas. However, COVID-19 pandemic and related lockdown has brought public mental health to the attention of president. He signed an Executive Order which allocated funds and established a working group to assess mental health challenges and take action for improvement (White House, 2020).

President Obama paid considerable attention to population mental health. He extended federal mental health parity to Medicaid plans and to insurance place marketplaces. Additionally, he enacted federal policy which provided housing to individuals who have mental illnesses and disabilities (Hart, 2016).

 

Population mental health was one of the major focuses for President Bush. He provided funds and enacted laws and policies. One of the major steps was establishing a federal law according to which health insurance for mental health became equivalent for other medical conditions. Additionally, the policies prohibited discrimination towards people who have a mental illness.

Identify the allocations of financial and other resources that the current and two previous presidents dedicated to this issue.

According to recent report of White House, the president allocated around $425 million to emergency funds for people with mental disorders.

 

The president allocated around $500 million in funding for mental health issues.

 

Around $126 million was used for Center for Substance Abuse Treatment. There were other programs involving more money, too (Lake, 2017).

 

Explain how each of the presidential administrations approached the issue.

President Trump cut the budget for mental health in the beginning of his Administration. However, in later period it became clear that mental health has critical importance, thus leading the President to pay more attention to it.

President Obama highlighted the importance of mental health in his activities, thus supporting any initiative aimed at improving population mental health. He generally continued (instead of cutting) the budget allocation for mental health.

President Bush considered mental health to be crucially important, as he aimed to screen whole US population for mental health.

President Bush very well understood the importance. It would be enough for his team to provide a recommendation and the President would consider the issue for his agenda. It would stay on agenda by the time there was an improvement in population mental health.

Narrative with the Facts

Policy makers and legislators spend time, finances and human resources to develop policies that aim to improve population health. The polices generally include healthcare insurance, cost mechanisms, healthcare system functions etc. However, the changes in social and economic life has made policy makers to reconsider the scope of policies. One of the most popular scholars in healthcare system Richard Saltman has questioned whether traditional pillars of healthcare system are in need of fundamental restructuring.

COVID-19 pandemic once again provided the need for mental health initiatives and policies. The focus of policy makers should expand to include mental health. Along with physiologic health, mental health is important for population to function in a way that will benefit the country and society. Lake and Turner (2017) consider that current treatments of mental health do not sufficiently address the mental health issues, leading to increase in the number of people facing numerous problems. Consequently, there should be a new and updated focus on mental health and policy initiatives that improve it. It should be included in the agenda for legislation as the population has suffered from the pandemic and many other historic events, having severe negative impact on overall well-being of people.

Nurses have critical role in all activities of patient care. They not only perform basic activities of care, but also provide emotional support to patients. They are very close to patients, which allows them to understand patient needs better than anyone else in the value chain of healthcare system. It will be very important and impactful to involve nurses in decision making, as they can provide advice on types of mental health issues faced by patients and probable ways of addressing these issues. Nurses have abilities and increasing enthusiasm that can contribute to the overall improvements in healthcare system (Dickens, Ion, Waters, Atlantis & Everett, 2019).

 Conclusion

            While Bush and Obama paid considerable attention and allocated resources to mental health, Trump cut budget on initiatives aimed at improving mental health. However, COVID-19 has lead Trump to understand that population mental health is critically important and allocated more funds. During Presidency of each President, there were events and situations that highlighted the need for mental health. In any case, the presidents should be involved in policy initiation and use their teams to improve population mental health.

 Week 6 | Part 6: Finalizing the Plan

⦁ I have considered various options for my nursing specialty, including a close look at my selected (or currently preferred) specialty and second-preferred specialty. I have also developed a justification of my selected (or preferred) specialty. Lastly, I have examined one professional organization related to my selected or preferred specialty and considered how I can become a member of this organization.

The results of my efforts are below.

 Step 1: Comparison of Nursing Specialties


Labor and delivery (L&D) nurse specializes in helping women during the entire birthing process. The work spans around pregnancy of women and delivery, including antepartum, intrapartum, post-partum and neonatal stages (Kovacs, 2021). The job of L&D nurse involves work and constant communication with not only the patient but also family members, providing them with support and assistance (Kovacs, 2021). For being able to work, L&D nurses need a license of a registered nurse (RN). There is no need to obtain other certification to work in practice, however, taking part in trainings and having certificates can increase the chances of finding a job. Also, trainings and other opportunities can help to become more skilled and experienced.
Psychiatric-Mental Health Nurse Practitioners (PMHNP) help patients to overcome psychiatric disorders and illnesses. The issues that patients face can include mood disorders, substance abuse, depression, etc. PMHNPs can work in a number of settings, including private entities, correctional facilities and mental health facilities. Being a PMHNP, requires to have a master’s degree. For becoming a PMHNP, licensure of RN is also required.
When deciding which specialty to choose, I considered which profession will help me to have more impact. Both specialties are important and support patients, however, I prefer PMHNP. Mental health is becoming a real issue for many patients and I believe I can have my contribution in that area. I already have an experience of working with patients needing mental health treatment, which allowed me to make my final decision.


Step 2: Justification of Nursing Specialty


I consider communication and patience as keys to working with patients needing mental health treatment. My previous experience has showed me that I both master communication and have sufficient patience to work with patients. There are many people experiencing stress, depression and anxiety which eventually can have long-term negative consequences on the lives and health of people. Mental health is one of the growing concerns in healthcare now, requiring more professionals to have relevant jobs (Lake & Turner, 2017). As a result, I consider my skills to suit better to the requirements and the role of PMHNP.

Step 3: Professional Organizations


American Mental Health Counselors Association is related to my preferred specialty. The mission of organization is to advance the professions associated with mental health and set standards for collaboration, advocacy, research, ethics, education and professional development (AMHCA, 2021). It is possible to join this organization through seven different membership types, which are clinical, regular, student, emerging, associate, retired, and unified dues. Each professional needs to choose the most suitable membership type and submit an application. Each membership type has a fee, starting from $199 for students.

Even though the response rate for study is high, nurses who responded are not always eligible. Nurses work in a stressful environment and they need support to stay mentally and physiologically healthy. This research focuses on a topic related to mental health.
The study identifies gaps in literature, thus creating a need for new studies. The location of study is limited to Northeastern academic medical center, which does not allow generalization of results to other locations and practice settings. The research focuses on an overlooked stress type – secondary traumatic stress. Leaders sometimes concentrate on direct stress and do not consider secondary stress as a potentially harmful risk factor for nurses.
The research is done in three tertiary care hospitals, which enable certain degree of variety in settings. The study uses cross sectional design, which assesses associations, rather than causal relationships. Labor and delivery nurses require special attention, considering the sensitivity and vulnerability of their work area. They communicate with patients which have specific needs and may be in very poor mental health condition. Thus, nurses should be able to take care of themselves in order to properly care for patients.


The study design has exclusion criteria (it excludes nurse managers), aiming to focus only on nurses as a sample participant. The method of STSS can create a room for recall bias, which happens when nurses reflect on their previous experiences.

Most of the questions are multiple choice or likert scale type questions, which enable objective data analysis.
The research provides important and insightful information on secondary traumatic stress, thus providing a base for new interventions aimed at decreasing the risk of stress. The study provides important clinical nursing implications as well as suggested clinical implications, which create a need for additional research.

Step 2: Summary of Analysis


I will recommend the use of this article to inform professional practice, because it provides accurate and reliable evidence. It analyzes other peer reviewed articles and synthesizes information derived from that studies. Then it conducts its own research and provides quite insightful practical implications. The study creates a new need for research, but also contributes to the research on secondary traumatic stress.
For the identification of peer-reviewed research, Walden university library has been used. The library has many databases, and the search tool allows finding any type of article in a matter of seconds. I used keywords related to my topic of interest and chose some filters. The first filter was date, as I wanted the article to be no more than five years old. The second filter was choosing only “peer reviewed” articles. For the analysis of peer-reviewed research, a simple approach was used. Before choosing the article I read the abstract or any summary that was available. After choosing the article, I read it from beginning till the end, skipping the literature review part. Once I understood the purpose, research design and major findings, I read once again in a greater detail. I downloaded the article as a pdf and highlighted important sentences when reading. The major focus was on study design, results, discussion and clinical implications. I read certain parts several times to make sure I did not miss a key point.
One strategy for finding a peer-reviewed research is using filters. The search tools generally allow using filters, thus making it easier to find the desired article. Another strategy for finding peer-reviewed research is using special databases that include only peer reviewed research. These databases and resources may require a monthly fee, but they provide access to high quality articles. When researching for articles, I found a good source of articles, which is called “ScienceDirect”. It provides access to peer reviewed journals, articles, book chapters and open access content.

 Part 1: Writing Sample: The Connection Between Academic and Professional Integrity


Students engage in various writing processes in which they are required to analyze, explain, and synthesize already existing information. Using the research in one’s work should be properly cited to avoid plagiarism and follow students’ ethical standards. Integrity, in general, relates to possessing good moral character, be honest, and uphold ethical values. Academic integrity has a fundamental role in education and acts as a foundation for preparing successful students (Keener, Peralta, Smith, Swager, Ingles, Wen & Barbier). Academic integrity means being honest, trustful, respectful, responsible, and fair in academic work. It relates to all aspects of academic life, including writing scholarly papers, submitting assignments, participating in discussion forums, communicating with other students, etc. Given a large amount of writing that students have to do, academic integrity becomes essential. When writing, students should be honest when using others’ work and cite it properly. It is disrespectful to use others’ work as one’s own. It is also unfair and distrustful to copy other authors’ work and present it to the professor as the original. 
Professional practice reflects the commitment of healthcare employees to provide patient care by upholding ethical values, establishing caring relations with the patient and patients’ families, being accountable to use specialized knowledge, and basing decisions on evidence. Glassman (2016) notes that professional practice gives meaning to the work of nurses and reminds them that patient and their families are at the center of practice. Scholarly ethics is interconnected with professional practice. It relates to moral and academic integrity when performing research. An individual who follows the values of scholarly ethics has a big potential to be successful in professional practice. Following academic integrity and moral standards, allows an individual to act in a responsible, fair, and honest manner. In professional practice, these values are highly important, considering the big importance of patients and their families. Also, the nurse collaborates with other healthcare professionals, which also requires establishing and maintaining honest relations. If the nurse fails to act responsibly and honestly when providing patient care, then the quality of care can be strongly impacted in negative terms. 
Numerous tools help students to improve their coursework. Grammarly helps to identify grammatical errors and improve the quality of work before submitting it to the professor. It allows the student to realize errors, correct them, and learn from mistakes. Safe Assign, on the other hand, shows which parts of work are close to containing plagiarism. It checks whether the student took words, phrases, or sentences from other authors’ work. As a result, the student can identify errors and make sure the work was written fairly and honestly, following the standards set by academic integrity. Paraphrasing, for example, is a technique to combine others’ words with their understanding. It helps to decrease the chance of submitting plagiarized work, though citing the primary author is essential. If the Safe Assign score is high, then the student did not take advantage of paraphrasing. Thus, it is a warning sign for the author to paraphrase or modify sentences and use in-text citations.

PART 2: Strategies for Maintaining Integrity of Work


One strategy for maintaining integrity and ethics of academic work while a student of the MSN program is to spend a considerable amount of time on student initiatives and course work. I am not going to leave papers and other work to the last minute. Instead, I will start planning each week on Sunday and work on papers as soon as I can. Time will allow to create better results and turn in papers that have higher quality. Time will help to reread and reconsider different parts and have multiple drafts before finalizing the work. Another strategy is to use Grammarly and SafeAssign tools to improve the quality of work and make sure I express high academic integrity. Before turning in each work, I will use these tools and create new drafts. Additionally, I will use the resources provided by the university to further increase the quality of the final draft.
⦁ To maintain integrity and ethics in professional work as a nurse, I will prepare a brief checklist and keep it on my phone and in my daily notebook, if needed. The questions will include five components of maintaining academic integrity: fairness, honesty, responsibility, trust, and respect. For example, one question will be: “Is it fair to your colleagues?” When I face a dilemma at work and do not know how to act, I will quickly consult the checklist. If the answers are definitely “Yes”, then I will act. If the answer is “Maybe” or “No”, I will rethink how I should proceed with my decision.
